If you are a DOC counselor, TOP can be a resource for you by providing a releasing inmate on your caseload with a packet of information and resources specifically for him, in the county he is releasing to.

To request a packet of resources specific to your offender, start with the Offender Questionnaire.

The questionnaire has all the information we will need to build a completely personalized packet for your inmate. You can email the questionnaire to us or it can be physically mailed to us at the address at the bottom of the website.

Please Note: We do not have "general" packets.
